Semaglutide: The Benchmark Incretin API

How a 31-amino-acid analogue of a gut hormone became one of the most-studied active pharmaceutical ingredients in modern medicine.

The short version

Semaglutide is a lab-engineered version of a hormone the gut already makes, called GLP-1. It is FDA-approved for type 2 diabetes, chronic weight management, lowering cardiovascular risk in people with heart disease and excess weight, and, since 2025, a liver condition called MASH. It comes as a once-weekly injection or a once-daily pill.

In its largest weight-loss trial, people taking semaglutide lost an average of nearly 15% of their body weight over 68 weeks, versus about 2% on placebo [4]. In a trial of more than 17,000 people with heart disease, it cut the rate of heart attack, stroke, or cardiovascular death by a fifth [3]. It is not without downsides — nausea and other digestive effects are common, especially early on — and this page covers those honestly, alongside what has been tested and what has not.

What it is

As an active pharmaceutical ingredient, semaglutide is a 31-amino-acid peptide built on the backbone of human glucagon-like peptide-1 (GLP-1), sharing roughly 94% of its sequence with the natural hormone. Two engineered changes give it drug-like staying power: an amino-acid substitution at position 8 blocks the enzyme (DPP-4) that would normally break the molecule down within minutes, and a fatty acid side chain attached near position 26 lets it bind tightly and reversibly to albumin, the most abundant protein in blood. That albumin binding shields it from the kidneys and extends its circulating half-life to roughly a week — the reason it can be dosed once weekly instead of continuously, as native GLP-1 would require.

The oral tablet formulation solves a different problem: peptides are normally destroyed by stomach acid before they can be absorbed. It is co-formulated with an absorption enhancer that briefly protects a small patch of stomach lining, but oral bioavailability is still under 1%, so the tablet must be taken fasted, with minimal water, and separated from other food or medication [5].

Semaglutide activates the GLP-1 receptor throughout the body. In the pancreas, that boosts insulin release only when blood sugar is already elevated, and suppresses the glucagon that would otherwise raise it — a glucose-dependent mechanism that keeps hypoglycemia risk low when semaglutide is used alone. It also slows how quickly the stomach empties, blunting after-meal blood-sugar spikes, which is part of why nausea is such a common early side effect.

Its effect on appetite is mostly centered in the brain. Semaglutide reaches appetite-control circuits in the hypothalamus and brainstem, quieting neurons that drive hunger and activating ones that signal fullness, without measurably lowering how many calories the body burns at rest [6]. That central mechanism is what patients describe as reduced 'food noise' — less mental preoccupation with eating, not just physically smaller meals.

What the research shows

Weight management (STEP 1). In 1,961 adults with overweight or obesity and no diabetes, once-weekly semaglutide 2.4 mg produced a mean body-weight change of -14.9% at 68 weeks, versus -2.4% with placebo [4].

Cardiovascular outcomes (SELECT). In 17,604 adults with established cardiovascular disease and excess weight but no diabetes, semaglutide 2.4 mg reduced the combined rate of cardiovascular death, non-fatal heart attack, or non-fatal stroke by 20% relative to placebo (hazard ratio 0.80) [3].

Kidney outcomes (FLOW). In 3,533 adults with type 2 diabetes and chronic kidney disease, semaglutide 1.0 mg lowered the risk of major kidney-disease events — kidney failure, a large decline in kidney function, or kidney/cardiovascular death — by 24% versus placebo (hazard ratio 0.76) [2].

Cardiovascular outcomes in diabetes (SUSTAIN-6). In 3,297 adults with type 2 diabetes at elevated cardiovascular risk, semaglutide reduced the same three-part cardiovascular outcome by 26% (hazard ratio 0.74), though retinopathy complications were more frequent in the treatment group (hazard ratio 1.76) [7].

Head-to-head against tirzepatide. In a 72-week trial of 751 adults with obesity, semaglutide produced -13.7% mean weight loss, compared with -20.2% for tirzepatide at each drug's best-tolerated dose — a statistically significant gap in tirzepatide's favor [1].

Overall safety picture. A dedicated safety review characterizes semaglutide's risk-benefit balance in type 2 diabetes as favorable overall, dominated by mild-to-moderate, mostly transient gastrointestinal effects, with nausea reported in roughly a third of patients [5].

Reported effects, cautions & safety

The following patient-reported effects are compiled from public drug-review sites and patient-experience research. This is anecdotal, not clinical evidence — it is self-reported, unverified, and none of it should be read as a guaranteed result.

The most consistently described benefit is a sharp drop in preoccupation with food — people frequently describe the constant mental background noise around eating simply going quiet, often within the first couple of weeks. Reduced cravings for sugar and fried food, steady weight loss, and (among people with diabetes) tighter blood-sugar control are also commonly reported. A number of reviewers mention an unexpected drop in interest in alcohol.

On the downside, nausea is the single most reported complaint, sometimes escalating to vomiting, usually worst around dose increases and easing over one to two weeks. Distinctive sulfur-smelling burps, alternating constipation and diarrhea, heartburn, and next-day fatigue after an injection are all commonly mentioned. A smaller group reports food aversions, a metallic taste, or appetite suppression strong enough that they have to remind themselves to eat; some note hair shedding or a more hollow facial appearance months in, generally attributed to the pace of weight loss itself rather than the drug directly.

Separately, the clinical literature documents several cited cautions. Gastrointestinal intolerance during dose escalation is the leading cause of discontinuation in trials [5]. Because rodent studies showed thyroid C-cell tumors at high doses, semaglutide carries a boxed warning and is contraindicated in anyone with a personal or family history of medullary thyroid carcinoma or MEN-2, although dedicated human-data review has not established a clear thyroid-cancer signal in people [5]. Acute pancreatitis is a recognized class warning, though evidence remains too limited to draw firm conclusions [5]. Gallbladder disease risk is measurably increased, largely attributed to the rate of weight loss itself [5]. In SUSTAIN-6, diabetic retinopathy complications were significantly more common among patients with pre-existing retinopathy who underwent rapid blood-sugar correction [7]. Body-composition studies show a meaningful share of lost weight is lean muscle mass, raising a sarcopenia concern, especially in older adults [5]. And weight tends to return after stopping — participants in one extension study regained roughly 11.6 percentage points of body weight within a year of discontinuation, framing this as chronic rather than short-course therapy [5].
